Switch Kit Instructions

Step 1

  • Click here to print out a New Account Information Form. Fill it out and bring it to one of our convenient locations.
  • You will need to bring a form of identification, such as a valid driver’s license.
  • Click here for a list of other acceptable forms of identification.

Step 2

  • Stop using your old checking account.
  • Keep your old checking account open until all your direct deposits and automatic payments take effect and until ALL outstanding checks clear.

Step 3

Click here to print out an Automatic Payment Checklist. List all your automatic payments that you will need to change or set up to be deducted from your new account. Print out each of the forms below as applicable. Fill out the forms and mail them to each company that you have automatic payments or direct deposits with or bring the forms to any of our convenient locations and let us help you.

This is a dual purpose form that allows you to have your direct deposits or automatic payments redirected to your new First National Bank checking account.
